How Long Does It Take for GLP-1 Medications to Work? A Week-by-Week Timeline

Most people don't see real results for weeks on GLP-1 drugs. Here's exactly what to expect at each stage — and when to be concerned if nothing is happening.

The most common question new GLP-1 patients ask: “When will I feel something?”

The honest answer is that GLP-1 medications work on a slow escalation schedule — by design. The dose starts low to minimize side effects, then increases every four weeks. Real weight loss doesn't typically appear until weeks 8 to 12. Full effect takes six months or more.

Here's what the research says about what to expect at each stage.

Week 1–4: The Starter Dose Phase

Semaglutide (Wegovy) starts at 0.25mg weekly. Tirzepatide (Zepbound) starts at 2.5mg weekly. These starter doses are sub-therapeutic — they're designed to let your body adjust, not to produce weight loss.

What you may notice:

What to expect on the scale: Most people see 0–2 lbs of weight change in the first four weeks, mostly from reduced calorie intake. Don't read too much into this number either way.

Weeks 4–8: First Dose Escalation

Semaglutide escalates to 0.5mg. Tirzepatide moves to 5mg. You're still not at a therapeutic maintenance dose, but this is where many patients first feel meaningful appetite suppression.

What you may notice:

What to expect on the scale: Average weight loss of 2–5% of body weight by week 8. For a 200 lb person, that's 4–10 lbs. Some people see less; some see more. Both are normal.

Weeks 8–12: Consolidating Effects

Another escalation: semaglutide to 1mg, tirzepatide to 7.5mg or 10mg. By this point, most patients are experiencing consistent appetite suppression and regular weight loss.

What you may notice:

What to expect on the scale: 5–8% total body weight loss by week 12 is typical for patients on escalation schedules. The STEP 1 trial showed semaglutide patients lost an average of 6.1% body weight at 12 weeks, and the SURMOUNT-1 trial showed similar trends with tirzepatide.

Weeks 12–20: Reaching Maintenance Dose

Semaglutide reaches its maintenance doses (1.7mg, then 2.4mg). Tirzepatide escalates to 12.5mg or 15mg. By the end of this phase, patients are on the maximum approved dose.

What you may notice:

What to expect on the scale: 8–12% body weight loss is typical by week 20. In the STEP trials, semaglutide 2.4mg produced 15% average weight loss over 68 weeks — so most of the journey is still ahead.

Months 6–12: Peak Response Window

This is where GLP-1 drugs prove their value. The STEP 1 and SURMOUNT-1 trials both showed their most dramatic results between weeks 36 and 68. Tirzepatide's 22.5% average weight loss in SURMOUNT-1 was measured at 72 weeks — more than a year in.

Average results by 12 months:

What If Nothing Is Happening?

Some patients are non-responders or partial responders. If you've been on a maintenance dose for 12 weeks and lost less than 5% of body weight, talk to your provider about:

The Long Game

GLP-1 medications are chronic disease treatments, not short-term fixes. The BMJ meta-analysis of 9,341 patients found that when patients stop GLP-1 drugs, most return to their original weight within about 1.7 years. That means the decision to start is really a decision about long-term management.

Plan for a 12–18 month commitment to see peak results. Be realistic about the first 8 weeks — they're about tolerating the drug, not transforming your body.
